It appears the doc has been hit by this bug: https://productforums.google.com/forum/?fromgroups#!topic/docs/K5wuV2676fs ... and therefore, at least for me, about 1/2 the comments don't appear on the document. They do appear in the comments list, but that makes them easy to miss for a new viewer. If you want to make sure you've seen all the comments check the comments list. The workaround is for everyone to close the document and wait 30m or so, so if you're finished with the document for the day please close it. If everyone closes it before going home (or wherever) the comments apparently should be back tomorrow.
